Description

As the hems caused by injuries and postoperative hems — are unpredictable. Formation of a hem depends as on the mechanism of initial damage and skill of the surgeon, and on the features of process of healing characteristic of your organism.

Many factors, including the size and depth of a wound, blood supply of area of damage, thickness and skin color and the direction of a hem exert impact on roughness of a hem. Besides, it is unconditional, the cosmetic problems caused by appearance of a hem are perceived by each person in own way.

Though no hem can be removed completely, plastic surgeons can often improve appearance of a hem and make it less noticeable way of an injection or putting medicines or by means of surgical intervention which is called "audit of a hem".

If you consider option of audit of a hem, here you will find the main information on the most widespread types of hems, procedures which are used for their treatment and the expected results.

Though audit of a hem is usually safe, surgical intervention always bears risk of emergence of complications. The infection, bleeding, reaction to anesthesia or repeated emergence of an ugly hem concern to them.

Keloidny hems

Keloids are reinforced, wrinkled, itching sites of cicatricial fabric which expand out of limits of edges of a wound or a section. They often have red color or more dark color in comparison with surrounding skin. Keloids arise when the organism continues to produce rigid fibrous protein — collagen — after healing of a wound.

Keloids can be formed in any area of a body, but most often they arise over a breast, on lobes of ears and on shoulders. With age the tendency to emergence of keloids decreases.

Keloids often treat injections of steroid means directly in cicatricial fabric for reduction of redness, an itch and burning.

If treatment by steroids appears insufficiently, cicatricial fabric it is possible to excise and close a wound one or several layers of seams. Usually this procedure is carried out in an out - patient order under local anesthesia, and after it it is possible to return for work in one - two days. In several days seams are removed. Skin transplantation is sometimes used, but in that case emergence of a keloid in the place from where take a skin transplant is possible further.

Irrespective of the chosen treatment method keloids have a persistent tendency to recurrence, sometimes reaching still the big sizes, than before treatment. For prevention of it the surgeon can combine removal of a hem with injections of steroids or direct application of steroids during surgical intervention. You can ask to carry the squeezing bandage around area of a hem during the period duration about one year. Even in these cases the keloid can be restored that each several years can demand repeated performance of procedures.

Hypertrophic hems

Hypertrophic hems often confuse to keloids as both that and others usually have an appearance of the reinforced formations of red color raised over a skin surface. However, hypertrophic hems remain within borders of an initial surgical section or a wound. They often pass in itself — though it can occur about a year or longer — or by means of local application or injections of steroids.

If conservative methods of treatment are inefficient, hypertrophic hems can often be improved in the surgical way. The plastic surgeon deletes surplus of cicatricial fabric and can move a section so that healing led to less noticeable hem.

Contractures

The burns or other damages leading to loss of a site of skin of the big area can heal with formation of a hem which pulls together edges of skin among themselves. This process is called "counteraction" ("tightening"). The turned - out contracture can exert impact on the next muscles and sinews, limiting the normal movements.

Correction of a contracture usually includes excision of a hem and replacement with its skin transplant or rag. In certain cases the technique under the name "Z - plastic" can be used. Also more and more important role is played, in particular, by techniques of stretching of fabrics. If the contracture existed certain time, after surgical intervention the physical therapy can be necessary for restoration of full function.

Hems on a face

Because of the localization the hem on a face often is considered a cosmetic problem irrespective of, it is hypertrophic, or not. There are several ways to make a hem on a face less noticeable. Often it is just excised and sew up a wound with very small seams, leaving thinner, less noticeable hem.

If the hem settles down along natural folds of skin, the surgeon can sometimes move it so that it was parallel to these lines and was not evident. (see "Z - plasticity")

Some hems on a face can be softened by means of laser techniques.
